Sometimes going to visit your pediatrician can be an overwhelming experience. By planning the visit ahead of time your can take full advantage of the face to face time with your pediatrician. Here are some recommendations:

1. Complete our VISIT PREPARATION FORM. (Remember to bring it to the appointment)

2. If you are coming for a CHECK UP or a PHYSICAL complete the appropriate developmental assessment for the age of your child. It will help us determine if your baby or child is developing appropriately.  Again, remember to bring the completed questionnaire.
